I don't know much about this guy but one of the biggest issues is the stigma around mental illnesses. Most can be treated very effectively but people are still hesitant to even get diagnosed because of how others will then view them, along with the consequences that come after being treated for a mental illness.
So most of the time a mental illness will go undiagnosed, which is then impossible to track even in background checks.
Even some extremely treatable illnesses like mild depression have unfortunate consequences when you get diagnosed and treated. My eldest stepdaughter was diagnosed with situational depression and was treated because she wanted the help. The problem now, she doesn't qualify for life insurance! That devastated her because she's lost family members very young and knows my situation and doesn't want to leave her husband in financial difficulty should something happen to her.
Obviously this guy had some issues, and people he knew even admitted they thought something was wrong with him. But unless he's been diagnosed and treated there is no way for anyone to know!

You are running with an anti-capital-punishment doctrine which has caused flawed logic (and circular reasoning).
You chose The Economist's Democracy Index because it shares your view/bias.
"Democracy" can be defined in many ways and I'd say The Economist is running with what many would call "Social Democracy".
For example, the fact a country employs capital punishment would be enough to push a country here from .799 Flawed to .80 Full.
(and for all we know, perhaps to .90)
So yeah, there is irony here, and much more.
You're using a source that penalizes level-of-democracy due to CP... to try to prove CP is not civilized!

Using their definitions/factors, USA is right up there with all the other top countries in level-of-democracy.
Anyway, employing capital punishment does not make a country less civilized... unless one holds that particular bias.

BTW The Economist concludes that less than 5% of the world’s population currently lives in a “full democracy”
... and that 89 countries experienced a decline in their total score over the last year, while only 27 saw an improvement.
A lot of this has to do with less trust in the electoral process around the world.
